This classification helps the radiologist to categorize each cystic renal mass as "nonsurgical" (ie, benign in category 1 and 2) or as "surgical" (ie, requiring surgery in category 3 and 4).

The Bosniak Classification discriminates between five different types of cystic renal lesions that are arranged in an increasing likelihood for malignancy. Bosniak type I and II lesions are benign in almost 100%of the cases, whereas Bosniak type II F lesions show a potential malignancy rate of 6% and followup needs to be performed. Source: Litzlbauer H. The Bosniak renal cyst classification system was initially reported in 1986, using CT scan findings. the Bosniak classification has been widely adopted and accepted because it addresses a difficult clinical problem and is easy to apply by both 2017-06-01 · The Bosniak classification system (BCS) for cystic renal lesions was developed based on CT findings . The BCS is reader dependent and prone to inter- and intra-reader variations [2] , [3] , [4] . Israel et al.

The process of categorizing renal cysts may be challenging, involving a series of decisions that may affect the final diagnosis and clinical outcome such as surgical management. The Bosniak classification system is not intended to be used alone to guide the management of patients with complex renal cysts. Management of complex renal cystic lesions is dependent on the individual patient’s combination of imaging findings, clinical factors, and available treatment options [ 16 ]. According to the classification, cystic renal lesions can be divided into subtypes with a low risk of malignancy (Bosniak I-II, nearly 0%) and subtypes with elevated rates of malignancy: Bosniak IIF (“F” = follow-up), Bosniak III, and Bosniak IV with a risk of malignancy of approximately 5%, 50%, and 100%, respectively [5,6,7].
